Failure of Service

  1. Failure of Service

    When any or all of the resources paged to an incident are unavailable.  If there is a delay of more than five (5) minutes (beyond the normal expected reaction time) to get the appropriate resource or appropriately trained personnel.

    Administration will make an assessment of the failure and score these events based on severity, bad outcome, or potential risk.  Please provide enough detail to allow the event to be understood.

    This information will be used to inform the conversation about needed staffing.
